Hola amigos, hoy os presento el caso de un hombre de 33 años que acude a urgencias por dolor en hipocondrio derecho de una semana de duración.
¿Cuál sería vuestro diagnóstico?
1. Infarto omental

Hallazgos: Estructura ovalada de densidad grasa en el borde antimesentérico del colon (flechas rojas en A y B), de unos 8 cm de diámetro, con un anillo hiperdenso periférico (flechas blancas) y una banda hiperdensa en su interior (flecha amarilla en B), asociado a afectación de la grasa adyacente y discreta cantidad de líquido libre en fascia lateroconal adyacente (B, asterisco).
Diagnóstico diferencial: La grasa del omento puede aumentar de densidad por cualquier proceso de vecindad (colecistitis, apendicitis (C, flecha), colitis, diverticulitis derecha), pero coexisten signos radiológicos de afectación de estas estructuras, no visibles en este caso.
La apendagitis epiploica es de menor tamaño, y mucho más frecuente en el lado izquierdo, en el borde antimesentérico del colon (D, flecha amarilla).

En un paciente con dolor en hipocondrio derecho, con leucocitosis y fiebre, el diagnóstico clínico es amplio. Lo más frecuente es una colecistitis, pero el hecho de ser afebril ha de hacer pensar en infarto omental, especialmente, en jóvenes.
La existencia en TC de una lesión ovalada, grande (> 5m), de densidad predominantemente grasa, en el borde antimesentérico del colon transverso o colon derecho, orienta a infarto omental.

– Es una necrosis segmentaria de la grasa del omento
- Causas:
- Torsión del omento
- Insuficiencia venosa secundaria a:
- Trombosis de venas omentales
- Cirugía
- Traumatismo
- Clínica:
- Dolor abdominal hipocondrio-vacío derecho
- Leucocitosis
- Muy raramente: náuseas, vómitos, fiebre
– Dos tipos:
- Primario
- Típico de niños
- Factores predisponentes: variantes anatómicas del omento o de su vascularización. Hipoaflujo omental (p.ej. corredores de maratón)
- Secundario
- Adultos. Algo más frecuente en hombres que en mujeres
- Relacionado con: Cirugía abdominal / Trauma / Masa abdominal / Obesidad
– Localización típica:
- Cuadrante superior o inferior derecho (porque el omento derecho es más largo y móvil)
Los hallazgos de imagen que permiten el diagnóstico de esta entidad son:
- Hallazgos ecográficos (E):
- Masa hiperecoica, (flechas rojas) ovoidea, no compresible en el punto de máximo dolor o donde se palpa la lesión en la exploración física
- Hallazgos de TAC (F y G):
- Masa omental solitaria y grande (mayor de 5 cm) de atenuación heterogénea y contenido graso (flechas rojas) que no capta contraste, de localización característica anterior al colon transverso o anteromedial al colon ascendente (borde antimesentérico).
- Suele disminuir de tamaño con el tiempo. Se recomienda un control TC al mes

Es un proceso inflamatorio focal, y clínicamente, dependiendo de su ubicación y la ubicación del dolor, puede confundirse con otras etiologías, por tal motivo, se presenta como diagnostico de segunda línea. No obstante, para nosotros es una lesión con imágenes muy características.
