Hola amigos, el caso de hoy corresponde a un varón de 53 años con aumento de tamaño de ambas mamas.

Hallazgos: Se observa tejido mamario en ambas mamas (ginecomastia). Se ve un nódulo en mama izquierda de bordes mal definidos (A, B flechas). En la mama masculina no se forman lobulillos y por eso una lesión lobulillar como el fibroadenoma puede descartarse. El aspecto radiológico sugiere un proceso maligno, confirmado con punción-biopsia.
Diagnóstico final: Ginecomastia y carcinoma de mama izquierda.
La patología mamaria en el varón es poco frecuente, pero existe. El propósito de esta presentación es mostrar los procesos más comunes de la mama masculina.
La causa más común de estudio de la mama masculina es la palpación de un nódulo o la apreciación de un aumento de tamaño de la mama uni o bilateral.
La patología más frecuente de la mama masculina es la ginecomastia, producida por el desarrollo del tejido mamario. La más frecuente es la ginecomastia puberal, que suele desaparecer espontáneamente entre 6 y 18 meses, aunque puede verse en todas las etapas de la vida.
El aspecto radiológico de la ginecomastia es variable, pero en todos los casos encontraremos un aumento de la densidad mamaria, producida por la presencia de mayor o menor cantidad de componente fibroglandular. El tejido mamario puede disponerse de forma pseudonodular detrás del pezón (fig.1) o tener una distribución más extensa y adoptar el aspecto de la mama femenina (fig. 2).

El aumento de tamaño de la mama puede ser debido únicamente a depósito de grasa, frecuente en pacientes obesos (pseudoginecomastia) (Fig. 3A). El aumento también puede deberse a una hipertrofia del músculo pectoral principalmente en deportistas y pacientes que consumen anabolizantes para aumentar la masa muscular (Fig. 3B):

El cáncer de mama en el hombre es una enfermedad poco frecuente. Menos del 1% de los cánceres de mama ocurren en varones. La ginecomastia previa no supone un factor de riesgo. La edad media de aparición es más tardía que en la mujer situándose alrededor de los 60 años.
Como en el varón no se realiza screening, prácticamente todos los cánceres presentan una alteración clínica: lesión cutánea o masa palpable.
El aspecto radiológico del cáncer de mama en el varón es el mismo que en la mujer (Figs. 4 y 5):

El resto de lesiones mamarias son infrecuentes. La mayoría de ellas no son propiamente mamarias, sino que se originan en los anejos, tales como los quistes sebáceos (Fig. 6 ) y hematomas (Fig. 7) entre otros:

Los consejos de Dr. Pepe y de la Dra. Rabanal:
- La patología más frecuente en la mama masculina es la ginecomastia.
- Un nódulo mamario en un hombre mayor de 45 años ha de estudiarse para descartar un proceso maligno.
- Otras lesiones mamarias suelen originarse en los anejos.

Hay una ginecomastia bilateral y en la región retroareolar pecho izquierdo hay un nódulo denso, de márgenes probablemente espiculados, suggestivo de corresponder a una neoplàsia maligna de mama (carcinoma). La mayoría de nódulos de mama en un varón són malignos.
