¿Cuál sería vuestro diagnóstico?

  1. carly says:

Thanks for sharing this interesting case study on thoracic imaging. I found the discussion about the skin fold and minor fissure displacement really helpful for understanding diagnostic clues. Great job to Carly and Angela for solving it!

  193. petter says:

Great case! I’d lean towards option 2, neumotórax, based on the supine chest X-ray findings in an elderly patient presenting with altered consciousness. Looking forward to seeing the solution on Friday to confirm the diagnosis!

There’s a useful resource at box-breathing.org that offers guided box breathing exercises for those looking to improve their breathing habits. It’s a simple technique that can make a real difference in how we feel. Do you incorporate any specific breathing techniques into your wellness routine?

  277. Wow, that’s a tricky one! I definitely would have jumped to pneumothorax at first glance, especially with the patient being unconscious. Good point about checking for the minor fissure beyond the line though; I’ll have to remember that for next time I see something like this.

  278. ImageNav says:

  279. Jason says:

A great reminder that careful observation of vascular markings crossing the line is key to avoiding a misdiagnosis of pneumothorax. The comparison with the follow-up CT really solidifies the teaching point.

  280. This is exactly the inspiration I needed for Los casos de Dr. Pepe. Bookmarking this for my next home project!

  283. gamer-names says:

Interesting case study! It’s fascinating how the initial assessment can be so crucial in identifying the underlying issues. The subtle loss of volume in the left lower lobe along with the pneumothorax makes for a tricky diagnosis. Thanks for sharing this, it’s a great learning opportunity!

  285. Dlook says:

Great thoracic case breakdown. The way you walk through pattern recognition and differential narrowing makes the final reasoning very teachable for day-to-day reading practice.

  286. Dlook says:

Excellent teaching case. The sequence from imaging clues to final interpretation is very clear, and the explanation of why alternatives were excluded is especially useful for routine reporting practice.

  287. Wow, that’s a tricky one! I totally would have jumped to neumotórax at first glance. Good point about checking for the minor fissure extending beyond the line, I’ll definitely keep that in mind for future cases.

  288. love type 16 says:

I was totally thinking pneumothorax at first glance! It’s interesting how the minor fissure extending beyond the line helps rule that out, I wouldn’t have immediately noticed that. Good teaching point about not jumping to conclusions and potentially ordering unnecessary tests.

  289. love type 16 says:

Wow, that’s a tricky one! At first glance I totally would have jumped to pneumothorax, especially with the patient being found unconscious. I’m glad you pointed out the minor fissure extending beyond the line, I definitely missed that. I’ll have to remember that trick!

  290. love type 16 says:

Wow, that’s a tricky one! I definitely would have jumped to pneumothorax at first glance, especially with the patient being unconscious. It’s a good reminder to double-check for things like the minor fissure extending beyond the line – I’ll have to keep that in mind for future cases!

  291. Leonardo AI says:

That line—“the chest radiograph, after more than one hundred years, is still alive, but probably mistreated”—really hit home. When teaching plain films, do you find schematic redraws or consistent overlays useful for pointing out things like a lost silhouette or subtle fissure shift, or do they risk oversimplifying the case? I’ve been experimenting with keeping annotations style-consistent across a series of cases to help learners lock onto the same landmarks—curious how that aligns with the way you’ve presented your fifty-five cases.

  292. It’s a great mental workout to keep your brain sharp and flexible.

  304. AngleMuse says:

Love how the case is presented first and the answer later – it really forces you to think instead of just memorizing patterns.

  305. For radiology residents, this kind of step‑by‑step reasoning is gold because it shows exactly how an expert narrows the differential in real life.

  309. love type 16 says:

Wow, that’s a tricky one! I was totally leaning towards pneumothorax at first glance, but you’re right, that minor fissure being visible beyond the line is a key detail. I probably would have jumped to repeat the X-ray if I saw that in the ER, but that teaching point about sense is so important.

  310. Long words can help clear the board faster effectively.

  312. Alex Chen says:

Interesting case. Medical case discussions are especially valuable when they balance imaging detail with clear explanation, and this kind of post gives readers a lot to think through carefully.

  317. This chest case study by Dr. Pepe is a clear, clinically relevant example—especially helpful for visualizing anatomical relationships in thoracic imaging. The annotations make subtle findings easier to grasp. As someone who works with historical medical images, I’ve found tools like NanoGPT’s Photo Enhancer useful for clarifying older scans without altering diagnostic content—just sharpening details that support interpretation. It’s not about creating new data, but recovering what’s already there, much like careful radiographic analysis itself.

  326. blood-money says:

  330. A very fascinating clinical case. The imaging details provided are excellent for learning.

  331. Excelente caso, Dr. Pepe.

  346. This was a fascinating case! It’s so easy to jump to a more serious diagnosis like pneumothorax when faced with a supine chest X-ray, but your detailed explanation highlighting the vascular markings and the path of the minor fissure really clarifies why a skin fold is the most probable answer. It reminds me of how crucial careful observation is in diagnostic imaging, much like how a good AI guide can help organize and present complex information efficiently.

  347. bing says:

This case is a classic reminder of how skinfold artifacts can mimic pneumothorax on supine chest X-rays. Key differentiators—such as lung vessels crossing the linear shadow and the minor fissure extending beyond the suspected pleural line—are critical to avoid misdiagnosis. Meticulous analysis of radiographic anatomy beats hasty conclusions every time.

  348. A powerful lesson in clinical reasoning: critical thinking and basic radiological literacy could have obviated unnecessary CT scans here. For elderly patients with non-specific presentations, ruling out benign artifacts first before advanced imaging aligns with safe, cost-effective care and reduces avoidable radiation exposure.

  349. sight words says:

This ESR case is an exceptional teaching tool for trainee radiologists and clinicians alike. It highlights common pitfalls in interpreting portable supine chest X-rays—one of the most frequently used yet error-prone imaging modalities in acute care. Mastering these artifact differentiations directly improves patient outcomes.

  350. This case underscores the importance of avoiding over-imaging in clinical practice. Unnecessary CT scans not only add to healthcare costs but also expose vulnerable elderly patients to unnecessary radiation. Simple confirmatory steps (e.g., expiratory chest X-rays) should always be prioritized before advanced imaging when artifact is suspected.

  351. cat names says:

Interesting case — thanks for sharing it.

Based on the description (elderly patient, unconscious, supine chest X-ray), I’d lean toward option 2: pneumothorax, especially since supine films can make it more subtle and easy to miss compared to upright imaging.

Looking forward to the official answer on Friday.

  542. The supine chest X-ray makes it tricky to spot a pneumothorax—I’ve seen how easy it is to mistake a skin fold for free air. Curious if the loss of volume in the left lower lobe might actually point to an underlying collapse or effusion. Hope Dr. Pepe turns up soon!

  543. testyouriq says:

That chest X-ray case was interesting! I thought it might be a pneumothorax, but the skin fold tricked me. The explanation about supine positioning was really helpful for future reference. Great case!

  544. aigptimage2 says:

I think it’s a pneumothorax based on the supine chest X-ray described. The patient’s age and being found unconscious make that likely. Interesting case, hope Dr. Pepe is okay!

  546. Interesting case! It’s tricky without the lateral view. I’d guess it’s a pneumothorax because of the supine position, but skin folds can look similar. Looking forward to the answer!

  547. GreyBrooks says:

  560. I’m terrible at reading these, I definitely would have jumped to neumotórax immediately! It’s so helpful to see the explanation about the cisura minor extending beyond the line though, I’ll try to remember that for next time. Congrats to Carly on getting it right!

Interesting case! I thought it was a pneumothorax at first, but the supine position made me reconsider. The skin fold artifact is tricky. Can’t wait to see the diagnosis!

  563. Wow, that’s a tricky one! I definitely would’ve jumped to pneumothorax at first glance, especially with the patient being unconscious. Good point about checking where the minor fissure is, though – I’ll have to remember to look for that in the future to avoid unnecessary scans.

  564. Faye Powell says:

Really helpful walkthrough of the chest case.

  565. slime rng says:

Great analysis of Dr. Pepe’s chest case! I found Carly’s breakdown of the skin fold versus pneumothorax particularly insightful, especially noting the vascular markings and minor fissure. It’s a fantastic reminder of how crucial those subtle details are in X-ray interpretation. Thanks for sharing this solved case!

  566. Dora says:

  567. Wow, that’s a tricky one! I definitely would have guessed pneumothorax at first glance. Good point about checking if the minor fissure extends beyond the line, I’ll try to remember that next time I see something similar. I’m glad Carly got it right!

  569. This case is a great reminder of how important supine chest X-rays are in emergency settings when patients can’t sit up. In my practice as an emergency physician, missing a subtle pneumothorax on these views can lead to a tension pneumothorax if the patient is placed on positive pressure ventilation—so teaching careful interpretation of the ‘deep sulcus sign’ is a must.

  570. Wow, that’s a tricky case! I definitely would’ve jumped to pneumothorax at first glance. It’s good to remember to check for those tell-tale signs like the vessels crossing the line, though.

  571. I found the imaging findings in Case 12 really interesting – the way you walked through the differential diagnosis was exactly what I struggle with in my own practice. The correlation between the radiographic signs and the final diagnosis helped clarify things I usually overcomplicate. I’ve started using a similar systematic approach when reviewing chest films, and it’s made a difference. By the way, I used an album collage maker to organize my teaching files for residents – it’s been helpful for case presentation prep. Thanks for another great case!

  572. Interesting case! I was thinking it could be a pneumothorax, but the skin fold option also makes sense. The supine position can make it tricky. Thanks for sharing this challenge.

  573. Interesting case! The X-ray in supine position can make it tricky. I think it might be a pneumothorax because of how the lung looks. Can’t wait to find out the answer!

  574. ovucal says:
