Hallazgos: La radiografía PA de tórax (A) muestra una silueta cardíaca discretamente aumentada de tamaño. La placa lateral muestra un pectus excavatum (B, flecha) acompañado de una columna recta. Estos hallazgos disminuyen el diámetro anteroposterior del tórax y obligan al corazón a desplazarse, simulando cardiomegalia en la placa PA.
Diagnóstico final: Pectus simulando cardiomegalia.

Teaching point: El pectus excavatum ocurre en hasta el 0,1% de la población.
Una de sus manifestaciones es una falsa cardiomegalia. El error se evita obteniendo una radiografía lateral.
