Queridos amigos, para empezar el año os presento el caso de una mujer de 68 años que vimos el pasado mes de noviembre con tos y moderada pérdida de peso. ¿Cuál sería vuestro diagnóstico?
1. Tumor pulmonar
2. Tumor mediastínico
3. Tumor pleural
4. Nada de lo expuesto

Hallazgos: las radiografías de tórax muestran una gran masa en hemitórax derecho, bien delimitada (A,B, flechas). Aunque el tamaño de la masa hace difícil determinar su origen, el parénquima pulmonar o los vasos no están distorsionados, lo que habla en contra de un origen en el pulmón. El diagnóstico diferencial reside entre un proceso pleural o mediastínico. El TAC muestra una masa que capta contraste, adyacente al mediastino y a la pared torácica anterior (C,D, flechas). Mi opinión fue a favor de un tumor fibroso de pleura, mientras que el resto del staff apostó por un tumor mediastínico.
Diagnóstico final: timoma

Teaching point: es importante señalar que cuando las masas son grandes es difícil determinar su origen. Esta fue mi excusa para no perder cara frente al staff. Nadie es perfecto!

Masa voluminosa extrapulmonar; de origen pleura (tumor fibroso de pleura, mejor posibilidad)
